ABSTRACT

INTRODUCTION. Ear pathology is a frequent disease in our environment, associated with repeated ear infections, with the presence of tympanic perforation and cholesteatoma, which will determine the presence of much more accentuated lesions in terms of auditory evolution or local or systemic complications. OBJECTIVE. To determine the association between the presence of cholesteatoma and tympanic perforation in patients with chronic otitis media. MATERIALS AND METHODS. Retrospective analytical epidemiological study. Population of 4 733 and sample of 75 patients for cases and 75 for controls based on clinical histories taken from the AS 400 computer system, who attended the Otorhinolaryngology outpatient clinic of the Carlos Andrade Marín Specialties Hospital in the period from January 2018 to December 2019; Inclusion criteria for case group: Men and women aged 20 to 65 years, diagnosis of chronic otitis media, diagnosis of otic cholesteatoma. Inclusion criteria for controls group: men and women aged 20 to 65 years, no diagnosis of cholesteatoma. RESULTS. A strong relationship was observed between having tympanic perforation and the development of cholesteatoma with an OR value of 33,14 with a 95% CI of 31,94 - 34,34, thus proving the study hypothesis. It was determined that tympanic perforation is a risk factor associated with the development of cholesteatoma in patients with chronic otitis media, the prevalence of cholesteatoma in relation to age was 72% in patients aged 41 to 65 years, with greater predominance in women in 57,3%. DISCUSSION. The presence of tympanic perforation according to what was observed is a risk factor for the development of cholesteatoma, mostly linked to Chronic Otitis Media. CONCLUSIONS. It was confirmed that tympanic perforation is a risk factor in the development of cholesteatoma in patients with chronic otitis media, which demonstrates the need for updated and continuous management in patients with this ear pathology. Studies with larger samples are required to determine other risk factors such as sex, education level and age that could influence the development of cholesteatoma.

ABSTRACT

Abstract Background: Tacrolimus is used to prevent unaesthetic scars due to its action on fibroblast activity and collagen production modulation. Objectives: To evaluate the action pathways, from the histopathological point of view and in cytokine control, of tacrolimus ointment in the prevention of hypertrophic scars. Methods: Twenty-two rabbits were submitted to the excision of two 1-cm fragments in each ear, including the perichondrium. The right ear received 0.1% and 0.03% tacrolimus in ointment base twice a day in the upper wound and in the lower wound respectively. The left ear, used as the control, was treated with petrolatum. After 30 days, collagen fibers were evaluated using special staining, and immunohistochemistry analyses for smooth muscle actin, TGF-β and VEGF were performed. Results: The wounds treated with 0.1% tacrolimus showed weak labeling and a lower percentage of labeling for smooth muscle actin, a higher proportion of mucin absence, weak staining, fine and organized fibers for Gomori's Trichrome, strong staining and organized fibers for Verhoeff when compared to controls. The wounds treated with 0.03% tacrolimus showed weak labeling for smooth muscle actin, a higher proportion of mucin absence, strong staining for Verhoeff when compared to the controls. There was absence of TGF-β and low VEGF expression. Study limitations: The analysis was performed by a single pathologist. Second-harmonic imaging microscopy was performed in 2 sample areas of the scar. Conclusions: Both drug concentrations were effective in suppressing TGF-β and smooth muscle actin, reducing mucin, improving the quality of collagen fibers, and the density of elastic fibers, but only the higher concentration influenced elastic fiber organization.

ABSTRACT

Introduction: Protruding ears represent a common congenital deformity that can generate psychological implications from childhood. Several surgical techniques have been described to correct this defect; however, clinicians frequently observe the intercurrence of abrupt relief of the posterior crus of the anti-helix and a partial relapse of the original defect. The objective of this study was to demonstrate a surgical technique that minimizes these complications. Methods: Sixty-five otoplasties were performed at Mater Dei Hospital, Belo Horizonte, MG, between 1995 and 2015. A surgical instrument similar to a 16-cm hemostatic forceps was developed by the plastic surgery team. All patients were operated under local anesthesia and sedation, under the supervision of an anesthesiologist in a hospital setting. Results: Correction of the congenital defect was achieved in all cases using a common method that was performed using a simple, novel technique that facilitates surgery and obtains natural and satisfactory results. Conclusion: The proposed techniques for otoplasty of protruding ears are simple, faster, and require easily obtainable and affordable materials. Satisfactory esthetic results were also achieved without observing the stigmas arising from atrial cartilage rupture.

ABSTRACT

In this paper, we present a case of auricular deformity whose surgical correction is difficult, involving scapha-helical unit associated with prominent ear in a 25-year-old female patient. This is an uncommon congenital malformation of the ear, resulting in a flattened, straightened and folded helical rim over the scapha, compromising the helical appearance. The proposed surgical treatment involved posterior approach and conchal cartilage graft to the restoration of the helical rim contour in a single procedure. This new approach provides a fine result to the ear, particularly because it restores the scapha-helical unit with no scar on the surface of the anterior ear and also effectively treats the prominent ear.

ABSTRACT

Introduction: The ear is a structure of the face with important aesthetic value. The dimensions of the ear and the appearance of its anatomical components change with age, disease and cultural practices which can result in an aesthetically unpleasant form over time. The earlobe is an important anatomical part of the ear and is considerably influenced by the factors above. It is made of fatty tissue and skin and can undergo changes in its proportion and flaccidity, which may be improved by surgical procedures to make it more aesthetically pleasing, providing a more proportional or younger appearance. This article reports the case of a 30-yearold patient who considered his earlobes disproportionate in relation to the rest of his ears and underwent surgery to have them reduced. Methods: A simple and fast recovery surgical procedure was carried out delineating and resecting a part of each of the patient's earlobes. Results: The results proved to be satisfactory as the patient had more proportional earlobes in relation to the rest of his ears achieving aesthetic improvement. Conclusion: The technique described in this article to reduce hypertrophied earlobes proved to be efficient.

ABSTRACT

Nodular fasciitis is a rare benign lesion. Here we report a case of post-auricular nodular fasciitis, which was misdiagnosed by fine-needle aspiration cytology [FNAC] as pleomorphic adenoma. Physical examination of an 18-year-old male revealed a right post aural firm immobile mass. Radiology suggested the presence of a hypo-dense to iso-dense subcutaneous mass. The swelling was excised and sent for histopathological examination which suggested the diagnosis of nodular fasciitis. FNAC reported pleomorphic adenoma of unusual location should raise the suspicion of nodular fasciitis

ABSTRACT

Introduction: Gentamicin is an ototoxic drug affecting both auditory and vestibular cells. Reactive oxygen species might play important role in the molecular pathway of this ototoxicity. Aminoguanidine is one of the guanidine derivatives and is considered as an antioxidant therapy

Aim of the work: to investigate the possible protective effect of aminoguanidine on ototoxicity induced by gentamicin in guinea pig

Materials and methods: Twenty one adult male guinea pigs were used. They were divided into three groups, seven animals each. Group I: served as a control group. Group II: I.M. injection of gentamicin [80 mg/kg/day] was used for 14 days. Group III: animals received gentamicin in the same way as group II and then followed by aminoguanidine [100 mg/kg/ day] administration using a gastric tube for 14 days. At the end of experiment, the cochlea was excised and prepared for light [LM] and scanning electron microscope [SME] examination. Immunohistochemichal technique was done to detect iNOS. Morphometric and statistical studies were also done

Results: By LM and SME examination, the present study showed that aminoguanidine protected the structure of organ of Corti in the cochlea by decreasing the degeneration of hair cells and other supporting cells. It also showed a significant decrease in the iNOS immune-reactivity in organ of Corti, stria vascularis and bipolar cells in spiral ganglia compared to group II

Conclusion: Aminoguanidine is a promising and successful antioxidant therapy for ototoxicity

ABSTRACT

Auditory Neuropathy Spectrum Disorders (ANSD) is the most currently accepted term to define the previously called auditory neuropathy or auditory dyssynchrony. Its prevalence is difficult to estimate due to multiple etiologies (hyperbilirubinemia, infectious diseases and sensory-motor, hereditary and/or ischemic-hypoxic neuropathies). Some individuals with AN have little or no communication difficulties while others are functionally deaf. Within the diagnostic criteria abnormal auditory nerve response in ABR are observed absent or markedly abnormal while otoacoustic emissions (OAEs) or the cochlear microphonic (CM) remain present and normal. Within the management of DENA is suggested that some children with ANSD appear to benefit from amplification and auditory based intervention while others require CI or other implants for the most severe cases. The objective of this article is to review the literature on disorders of the auditory neuropathy spectrum.

ABSTRACT

This study was carried out to assess the association between of mycoplasmas species with ear mites Raillietia auris and R. flechtmanni in the external ear canal of 60 bovines at slaughter time from the State of Rio de Janeiro, Brazil. Steril syringes (60ml) loaded with buffer solution (PBS, pH 7.2) were used for the ear canal flushing. Were processed 218 mites for mycoplasma isolation. A pool of mites from each sampled bovine was washed five times sucessively in 1mL of liquid modified Hayflick´s medium. The washed mites obtained were diluted up to 10-1 at 10-5, inoculated in liquid and solid Hayflick´s media and incubated at 37ºC for 2-3 days, being the plates put into jar for the obtention of microaerofilia condition. The Typical colonies were typified by the indirect imunoperoxidase test (IPI) with paper discs satured with hyperimmune rabbit sera. In the studied bovine high prevalence was verified Raillietia spp. 76.7% (46/60). The parasitism by mycoplasmas and mites was verified in 40 animals (74.1%), this association was significant (p<0.001). Among the mites processed for isolation mycoplasmas 193 were female and 25 males. The frequency of Mycoplasma in Raillietia spp. was of 81.2% (177/218) (p<0.001). Of the females identified 52.3% (101/193) were R. auris and 47.7% (92/193) were R. flechtmanni. The frequency of Mycoplasma in the females of R. auris was of 75.2% (76/101) and 88% (81/92) in R. flechtmanni (P<0.05). The mycoplasmas species typified by IPI in the Raillietia auris mites were M. alkalescens 6.9%, M. arginini 3.4%, M. bovirhinis 9.2%, M. conjunctivae 18.4%, M. mycoides mycoides LC 8.0%, M. capricolum 5.7%. In the R. flechtmanni mites mycoplasmas species typified were M. alkalescens 12.2%, M. arginini 1.0%, M. bovirhinis 18.9%, M. bovis 2.2%, M. conjunctivae 21.0%, M. mycoides mycoides LC 11.0% e M. capricolum 4.4%. The species of identified mycoplasmas in the external ear canal bovine and mites were exactly the same. The results confirm that the external ear canal cattle's ear canal is also a mycoplasmas source, including potentially pathogenic species for cattle, and these mollicutes are closely related with mites Raillietia spp. that is carrier and this agent in your organism.

ABSTRACT

Thirty-seven species of common seaweeds from the coast of Korea were screened for anti-inflammatory activity Methanol extracts of the seaweeds were tested against mouse ear edema and erythema induced by phorbol myristate acetate. At 40 mg ml(-1) of extract, edema was strongly suppressed by the seaweeds Undaria pinnatifida and Ulva linza, with relative inhibition of 85 and 84%, respectively These two seaweeds also showed the greatest suppression of erythema, with inhibition of 78 and 70%, respectively IC50 values of U. pinnatifida were 10, 15, and 18 mg ml(-1) when inflammation symptoms of edema, erythema, and blood flow, respectively were measured. The IC50 of U. linza was 20, 26, and 31 mg ml(-1) when edema, erythema, and blood flow, respectively, were measured. A linear correlation among inhibition rates of edema, erythema, and blood flow was observed with high confidence.

ABSTRACT

Keloids are defined as dermal fibrotic lesions which are considered an aberration of wound healing process. Their etiology and pathogenesis are poorly understood. Different treatment modalities are described in literature and no one method has been found completely successful. In the present work we combined intralesional surgical excision followed by in-tralesional corticosteroid injections to improve the therapeutic outcomes. Thirty female patients with ear lobe keloids due to ear piercing, burn and trauma were included in this study. Keloids were excised intralesionaly followed by adjuvant intralesional injection of triamcinolone acetonide 3 weeks after excision. Injections were done once / week for 3 weeks, followed by another course of injections 6 weeks later. Cases were followed up every 2 months for one year: 2 cases [6.6%] had relapsed after 4 and 6 months post-operative respectively. Our results confirm the effectiveness of this technique in reducing the rate of recurrence of ear lobe keloids

ABSTRACT

Colliod milium is a degenerative change, which is characterized clinically by development of multiple yellowish, translucent papules and nodules on the light exposed skin and histologically by the presence of colloid in the dermal papillae. This is a rare condition and has some variants. We report a case of colloid millium with nodular and plaque lesions on his ears. Our patient was a farmer with a history of more than 47 years chronic sun exposure and showed photoaging

ABSTRACT

Dogs having aural hematoma need a quick clinical or surgical intervention which will allow the reestablishment of the physiological and esthetical condition of their ear. The present study approaches the procedures used in the draining technique of the aural hematoma in twenty dogs. All of those were submitted to the use of a drain, permitting the definitive reestablishment of the ear pinna without any deformations by contracting skin

ABSTRACT

Leishmania tropica and L. major are etiologic agents of human cutaneous leishmaniasis. Delayed type hypersensitivity (DTH) is an immunologic response that has been frequently used as a correlate for protection against or sensitization to leishmania antigen. In BALB/c mice, L. tropica infection results in non-ulcerating disease, whereas L. major infection results in destructive lesions. In order to clarify the immunologic mechanisms of these 2 different outcomes, we compared the ability of these 2 leishmania species in induction of DTH response in this murine model. BALB/c mice were infected with L. major or L. tropica, and disease evolution and DTH responses were determined. The results show that the primary L. major infection can exacerbate the secondary L. major infection and is associated with DTH response. Higher doses of the primary L. major infection result in more disease exacerbation of the secondary L. major infection as well as higher DTH response. L. tropica infection induces lower DTH responses than L. major. We have previously reported that the primary L. tropica infection induces partial protection against the secondary L. major infection in BALB/c mice. Induction of lower DTH response by L. tropica suggests that the protection induced against L. major by prior L. tropica infection may be due to suppression of DTH response.
